Overview

Semax is a synthetic heptapeptide developed in Russia in the 1980s as an analogue of a fragment of the hormone ACTH. In its home country it is a registered medicine, sold as an intranasal solution and used for acute ischaemic stroke, chronic cerebrovascular problems and some cognitive indications, and it sits on Russia's List of Vital and Essential Drugs. In the West it has a second life as a grey-market "research chemical" prized by nootropics enthusiasts for supposed focus and verbal-fluency benefits.

Here is the honest shape of the evidence: the animal and mechanistic data are reasonably robust and consistently positive, while the human data are small, almost entirely Russian-language, weakly blinded, and concentrated on stroke rather than cognitive enhancement in healthy people. There are no large, registered, placebo-controlled Western trials, and it is not approved by the FDA or EMA [18]. A further caveat runs through almost everything below: the bulk of the research comes from a single interconnected Moscow research network, which raises real questions about independent replication and publication bias.
