Overview

AICAR (acadesine; 5-aminoimidazole-4-carboxamide-1-beta-D-ribofuranoside) is the compound that launched the phrase "exercise in a pill". The story is irresistible: a 2008 paper in Cell showed that four weeks of oral AICAR, with no training at all, raised treadmill endurance in sedentary mice by roughly 44% and switched on an exercise-like gene programme in muscle [1]. That single result is the engine behind almost everything written about AICAR in the performance and longevity space.

Here is the part the marketing leaves out. The endurance evidence is essentially all from animals. There are no human trials showing AICAR improves endurance, burns fat, or extends healthspan. Reviews of the field are blunt that AMPK has not been successfully drugged for metabolic disease in people [9][10]. Paradoxically, the strongest human data for this exact molecule come from a completely different world — heart surgery, where it was studied as "acadesine" — and from a small leukaemia trial. So the evidence is genuinely mixed: robust preclinical mechanism, real human safety and efficacy data in cardiac surgery, and a translational void where the performance claims should be. It is WADA-banned and approved nowhere.
